Ullensaker kommune

Gjerdrum, Hurdal, Nannestad and Ullensaker municipalities are cooperating on the establishment of the Upper Romerike local medical centre (LMS). Through this cooperation the municipalities shall provide local medical health services. The services shall, in accordance with legal requirements, include a 24 hour doctor emergency surgery with observation beds and municipal acute 24 hour treatment (KØH). In addition a reinforced short stay department will be established at the centre. LMS is to be established in a joint health centre.

Beskrivelse av opsjoner:
The OPS-project must make allowances for an expansion to the area required of up to 2 000 m².
The health centre can reckon on being expanded to be on a level with a local hospital for a future population in Romerike, which can increase from 100 000 to 140 000 by 2040 (40 %).

Requirement:
The contractor shall have his tax and VAT payments in order.
The contractor must be a legally established company.
Contractors who shall perform work in Norway must meet the requirements for health, safety and environment that the Norwegian authorities at any time establish.
Documentation:
Tax certificate and VAT certificate.
Company Registration Certificate.
HSE self-declaration.
Økonomisk og finansiell stilling:
— The contractor's annual financial statements, annual report and audit report for the last 3 years and recent information which is of importance to the contractor's fiscal figures. Financial statements, annual report and audit report are what Norwegian companies send to the Brønnøysund Register Centre.

— In the event of creation/utilisation of a project company (SPV), there must be confirmation that there will be a mother company guarantee or other guarantee from a third party (bank/finance) for the obligations the project company accepts until the start of the lease period.

Teknisk og faglig kapasitet:
1.Competence:
The contractor shall prepare a competence matrix for each area of responsibility/profession. The following areas of responsibility shall at a minimum be included in the matrix: (1) project management, (2) architect, (3) technical consultants (engineering designers), (4) financial consultants, (5) property manager and (6) any central contractors. An overview shall be given of available resources based on education and professional qualifications that the service provider can use within each of the general and special professions. For each individual area of responsibility, it shall be stated which company shall conduct the assignment (if sub-contractors are used). CV s are not to be submitted.

2. Experience from similar assignments:
The contractor shall answer the application form point 4.3. Here you must provide the most relevant assignments from the last five years, including their value, time frame and the recipient. It shall specifically state:
— Which projects the contractor both developed and is/was a long term occupier.
— Which projects may have been undertaken.
The contractor must provide a name and email address for a reference at the receiver for each of these most relevant projects, including tenants. The contracting authority reserves the right to contact the stated references.
1. Description of the site/property with the address and cadastral and property numbers.
2. A general description that states and refers to the planning base's possibilities and limits for the actual localisation and which justifies why the placement of the new health centre is in line with the planning base (simple feasibility study).
3. Transcript from the land register that confirms the availability of the property or documentation that the contractor has or will get for an unrestricted period the right to use the site/property for a health centre.
— The contractor shall present binding documentation, for example, a binding statement or collaboration contract, which states which actors are planned to be jointly housed in the health centre.
Valid proof of central certification for the right to be responsible in development class 3 for SØK and PRO respectively.

— Documentation that the company meets the requirements of local responsibility right for the initiative within the same development class and function as stated above. — A statement of the contractor's quality assurance/management system to ensure that the assignment is carried out in a most efficient and purposeful manner, including systems to ensure protection of interdisciplinary environment and climate requirements.

The contractor shall have a very good ability to fulfil the contract:
1. The contractor is required to have very good competence to carry out equivalent assignments (as a property developer and long term property owner with management, maintenance and operational responsibilities).
2.The contractor is required to have experience from similar assignments (property developer and long term property owner with management, maintenance and operation responsibilities).
The contractor is required to either be in possession of or will get possession of a suitable site/property in Ullensaker in accordance with the municipality's and the National Housing Bank of Norway's requirements and expectations for the location. The site must be located within the stated area border as this border is shown on the map enclosed as annex 2.

The contractor is required to join up with other actors located in the health centre, preferably health services.
The contracting authority would particularly like to facilitate the establishment of an x-ray institute and a laboratory in the health centre. In addition specialist health services, a doctor surgery, gymnasium can be jointly housed there.
The contractor is required to posses or be able to obtain necessary government approvals for completing the contract.
A good, efficient quality assurance system is required.
