Tenderers shall have sufficient economic and financial solidity to fulfil the contractual obligations.: Documentation: The last submitted Annual Financial Statements with the annual report and the auditor ́s report to the Brønnøysund Register Centre and any new information of relevance to the tenderer's economic and financial situation. Tenderers shall have their tax and VAT affairs in order.: Documentation: Norwegian tenderers shall enclose a VAT and tax certificate, which is not older than 6 months calculated from the deadline for submitting a tender. Any arrears shall be justified. The certificate can be ordered and obtained from Altinn, or ordered from the tax collector or the Norwegian Tax Administration. Tenderers shall manage the company in accordance with the regulations on management and quality improvement in the health and welfare service.: Documentation: Self-declaration and copy of (only) table of contents for the system ́s documentation. Registered in a trade register: Tenderers are registered in a professional register in the member state in which the tenderer is established. As described in annex XI of directive 2014/24/EU; suppliers from certain member states may have to fulfil other requirements in the mentioned annex. The tenderer shall be a legally established company. Documentation: Norwegian tenderers shall enclose an updated Company Registration Certificate. Companies under the foundation shall submit a foundation document. C.f. the tender documentation point 4.2.1.

Registered in a trade register or company register.: Tenderers are registered in a company register or a trade register in the member state in which the tenderer is established. As described in annex XI of directive 2014/24/EU; suppliers from certain member states may have to fulfil other requirements in the mentioned annex.

Services only: Only for services: In the reference period, the tenderer has carried out the following important goods deliveries of the requested type, or the following important services of the requested type. The contracting authority can require up to three years experience and allow experience from the previous three years to be taken into consideration.

Technical personnel or technical units: Tenderers can use the following technical personnel or technical units , particularly those who are responsible for quality control: For technical personnel or technical units that do not directly belong to the tenderer ́s business, but if the capacity the tenderer will use, cf. part II, section C, separate ESPD forms shall be supplemented.

Technical facilities and quality assurance measures: The tenderer utilises the following technical facilities and quality assurance measures and the tenderer ́s survey and research facility is as follows:

If the tenderer has not been guilty of the mentioned conditions, the tenderer is asked to sign a self-declaration on this. Cf. Annex 3 Tender Letter - point 1. Proper professional conduct. Tenderers must have experience with deliveries of comparable nature and scope requested by the Contracting Authority.: Documentation: Overview of relevant deliveries in the last three years, including the name of the contracting authority and the financial extent. Tenderers shall have an overview of serious incidents in their own business.: Documentation: Overview of authority inspections in the last three years. C.f. the tender documentation point 4.6.1 Minimum requirements for qualification requirements.

The tenderer ́s premises must be secured throughout the entire contract period.: Documentation: Documentation of ownership of the premises. Leased premises shall document a rent contract for the entire contract period, including an option for an extension. Recently established companies can document the requirement by a copy of an intention contract or equivalent documentation that shows that the tenderer is in a concrete process as regards premises within the health region for the entire contract period, including the option's year. C.f. the tender documentation point 4.6.2 Minimum qualification requirements.

Tenderers shall not have employed personnel who may have a conflict of impartiality with the health authorities in Helse Nord.: Documentation: Tenderers shall confirm that health personnel with main working conditions in Helse Nord have registered the biernity at the main contracting authority, cf. § 19, 2nd section. Executing personnel shall always have the authorisations and certifications required for the occupational group or the speciality.: Documentation: Self-declaration stating that the tenderer always ensures that personnel have authorisations/certifications that the occupational group in question/speciality requires. Tenderers shall be connected to the Norwegian health network, or from the start of the agreement have such connection for all the delivery addresses included in the tender.: Documentation: Documentation that the tenderer is affiliated to the Norwegian Health Network, or is in process regarding connection to the Norwegian Health Network.
