Trondheim Kommune
Trondheim municipality, hereafter called the Contracting Authority, invites tenderers to an open tender contest for a contract for eye tests and computer glasses for the municipality ́s units. The framework agreement shall cover Trondheim municipality with cooperation partners ́need for computer glasses and eye tests in accordance with the employer ́s obligations cf. The Working Environment Act § 4-2 (2) sets requirements for the employer to facilitate the formulation of each employee ́s work situation. The framework agreement shall cover Trondheim municipality with cooperation partners ́need for computer glasses and eye tests in accordance with the employer ́s obligations cf. The organisation, management and participation regulations § 14-4.The process at the Contracting Authority will be carried out as described in "Requisition - Eye examination for computer glasses", see annex 5. The form will be revised as compared to the contract with the new tenderer after the contract has been signed. The Contracting Authority requests tenders for optician examinations and computer glasses. This includes: Eyeglass glasses, completed and assembled: One-strength glass Multi-strength glass with invisible transition (This need must be documented separately in each case in accordance with the Norwegian Labour Inspection Authority ́s definition of computer glasses). Counter and room variable glass if needed for larger fields of view (This need must be documented separately in each case in accordance with the Norwegian Labour Inspection Authority ́s definition of computer glasses). Eye examination including pressure measurement, Eyeglass adaptation. The offered products shall be of satisfactory quality, but not above the quality that seems price-driven. The eyeglass frame shall have a good ergonomic design, it should be comfortable to wear and be of sober standard. Tenderers must describe in the tender what points are offered and on what terms. The Contracting Authority is expected to be a prioritised customer.

Requirement: The tenderer shall have the economic and financial capacity to carry out the assignment - the tenderer shall be credit worthy, Class A. Credit worthy with security is not accepted. A/N is sufficient for newly established tenderers. Documentation requirement: • Credit rating of the tenderer, not older than 6 months calculated from the deadline for receipt of tenders. The assessment shall be based on the last known fiscal figures with an indication of how the credit rating has evolved during the past three years. The assessment shall be issued by a company with a credit reporting license from the Norwegian Data Protection Authority, see www.datatilsynet.no The result of the credit rating shall be given as a graded value (letter or number) on a defined scale. The tenderer shall have authorisation as an optician in accordance with the Health Personnel Act dated 02.07.99 and employees who have further education within workplace optetry. Minimum qualification requirements
Requirement: Tenderers shall have a system and routines for reducing the most important environmental impact from their activities and shall use environmental management measures throughout the contract period. Therefore it is a requirement that the company has an environmental management system, such as Miljøfyrtårn, EMAS, ISO 14001 or equivalent.
If the entire company is environmentally labelled with a type 1 environmental label, such as the Swan Ecolabel or equivalent, this is also approved.
Documentation: Tenderers shall complete the attached Environmental Self-Declaration, as well as enclose a certificate of environmental management system.
