Helse Bergen HF intends to replace surgical tables in Helse Bergen; Haukeland University Hospital (the Central Operation Department (SOP) and the Women's Clinic), Voss Hospital and Kysthospitalet in Hagevik, KiH. Helse Bergen currently has 26 theatres with a fixed base and 5 theatres with a mobile base. This could be changed. The contracting authority envisages a need for approx. 2 tables with a trolley per theatre, like SOP is currently organised. The replacement is planned over 4 years. This is estimated to be a total of 62 tables. In addition KiH will need a special surgical table for back surgery.
Helse Bergen HF will see it as a big advantage if the table system is universal and have many applications, with optimum use within each of the different areas, a good selection in clay pillows and additional equipment, tolerate high patient weight in different positions, secure from operational interruptions and collisions, quick and safe operation with good regulation options and easy to work around the table.
The contracting authority would like tenders for three different system tables
1. Tables for a fixed base (approx. 50-52 tables)
2. Tables for mobile/movable base (approx. 10-12 tables)
3. Special surgical tables for back surgery (1-2 tables)
The contracting authority currently has baseplates from Maquet 1150 cast into the floor. Fixed bases ought to be compatible with this base. This applies to the current 26 base stations. See Annex 10.
If the existing baseplate cannot be used, the additional cost for a new baseplate is to be added to the calculation of the total cost. The estimated cost of replacing a baseplate is 40 000 NOK excluding VAT per unit. The cost is to be added to the total cost for any replacement in Part 2 Annex 1 Price Form.

The contracting authority would like tenders for complete, functional set-ups within the following main categories:
— Neuro surgery
— Plastic surgery
— ENT surgery
— Urological surgery
— Gastro/vessel/endocrine/general surgery
— Orthopaedic surgery
— Bariatric surgery
— Gynaecological surgery
— Special table for back surgery

The detailed planning of systems and the extent will be reviewed with the chosen tenderer.
The offered surgical tables shall be delivered as requested, with all the necessary accessories, delivered complete, assembled and ready for use.

Helse Bergen HF would like to receive tenders for 1 complete special surgical table for back surgery that can see to the sketched needs and wishes as described and requested here in the best possible way. Kysthospitalet in Hagavik, KiH, is a university hospital and special hospital for back surgery. They perform approx. 350 back operations per annum. The actual back table can be used for all of these operations, but it would be a great advantage for the surgical result to use a special back table in 100 operations. The contracting authority needs a back table that meets the challenges that exist in spinal surgery at KiH. The back table is important for patients with degenerative conditions that require long bracings, who must be operated on a table on which the correct lordosis can be achieved in the lower back when the bracing instruments are fixed. Furthermore, it is important to achieve good lordosis for a good clinical result for the patient. There is a strong scientific basis that states that the correct lordosis gives a better clinical result in these types of operations.

1. Option for a service contract. The contracting authority will be free to choose and enter into a service contract after the guarantee period has expired, c.f. Part 2 Annex 5 — Service Contract.

Norwegian tenderers are required to have fulfilled their tax and VAT payment obligations.

Certificate for tax and for VAT. Issued by the Norwegian Tax Administration.

The certificates shall be issued on the Tax Directorate's form RF-1316 or another form that provides the same information.

The certificates shall not show significant arrears. The certificate must not be more than 6 months old at the submission date of the tender offer.

Tenderers are required to be a legally established company.

The tenderer can document this by submitting a copy of their Company Registration Certificate. Foreign companies shall submit proof that the company is registered in a trade or business register as prescribed by the law of the country where the tenderer is established.

Økonomisk og finansiell stilling:

The supplier must have the financial capacity to fulfil the assignment/contract.

— Credit evaluation/rating, not older than 6 months, and which is based on the last known accounting figures. The rating shall be carried out by an officially approved credit rating institution.

We reserve the right to obtain a credit rating on our own initiative from a reputable rating company.

Companies without a satisfactory credit rating, newly started companies with a credit rating of AN, individual foreign companies or companies without reporting obligation to the Brønnøysund Register Centre will not always be able to submit a credit rating as documentation that the qualification requirement is satisfied.

In such cases, this can be compensated by providing alternative documentation together with the tender.

— Latest annual financial statement with notes including auditors certificate.

— Income statement and balance for the last six months if it is more than six months since the last annual financial statement.

Teknisk og faglig kapasitet:

The tenderer is required to have adequate experience from similar assignments. Similar assignments are seen as the delivery of such performance as encompassed by the competition for major public departments/companies or to major private corporations.

— Tenderers shall enclose a list of the most important relevant deliveries and services performed during the past 3 years, including information on the value, date, and recipients. Certificates issued or verified by the Contracting Authority can be attached.

Tenderers shall have good implementation ability/capacity.

— Tenderers shall enclose a description of their technical personnel or technical units they have at their disposition to fulfil the contract, regardless of whether they belong to the company or not, especially those responsible for the quality control.

Tenderers must have a well-functioning quality assurance system for the services that shall be provided.

If the routines described in the Tenderer's quality assurance /or quality control system are in accordance with ISO standards or equivalent third party verification system, it is sufficient to enclose a valid certificate with information on where this is described. Alternatively, an existing system and routines that substantiate that the requirement is fulfilled can be described.

Tenderers are required to have a well-functioning quality assurance system that ensures that the fulfilment of the contract has a low impact on the environment.

— Tenderers shall enclose a description of their quality assurance system. The description shall be a maximum of 6 A4 pages. If the routines described in the Tenderer's quality assurance /or quality control system are in accordance with ISO standards or equivalent third party verification system, it is sufficient to enclose a valid certificate with information on where this is described.
