The contracting authority shall have an option to extend the agreement With the following Products/and services:

1. Purchase of instruments for Helse Førde Hospital Trust

2. Purchase of instument(s) for Haraldsplass DiakonaleHospital (HDS)

3. Annual service agreement for the instruments after the warranty period is expired for Helse Bergen Hospital Trust, Helse Førde Hospital Trust og Haraldsplass Diakonale Hospital(HDS)

4. The option to enter into the agreement for instrument related consumables for Helse Førde Hospital Trust og Haraldsplass Diakonale Hospital (HDS)

For option 1 og 2 these have to be used no later than 31.12.2017.

Egnethet til å utøve yrkesvirksomheten:

A. It is a criterion that Norwegian tenderers shall have fulfilled its obligations in connection with payment of tax and VAT.

Documentation: Tax certificate. In Norway certificates are issued by the city or municipal treasurer. Tenderers from other countries must attach an equivalent certificate from the country in which they are incorporated.

2. Certificate for VAT payments. In Norway certificates are issued by Skattefogden (the Tax Office). Tenderers from other countries must attach an equivalent certificate from the country in which they are incorporated.

The certificates must be issued on form RF-1244 from Skattedirektoratet (the Directorate of Taxes) or another form that contains the same information.

The certificates must not show significant arrears. The certificates shall not be older than 6 months from the delivery date for the tender.

B. It is a criterion that the tenderer is a legally incorporated enterprise

Documentations: A certificate from Foretaksregisteret (the Register of Business Enterprises) or a certificate of registration in the comparable professional register in accordance with the legislation in the country in which the tenderer is incorporated.

Økonomisk og finansiell stilling:

A. The tenderer shall have the necessary economic capability to perform the contract

Documentation: Credit rating, not older than 6 months, and which is based on the last known accounts. The rating shall be performed by an accredited credit rating agency. If the Tenderer is unable to provide a credit rating, the Contracting Authority will obtain one on its own initiative.

If the Tenderer's credit rating is not satisfactory, for example where a newly-formed company with an AN credit rating is concerned, this can be compensated for with a suitable guarantee from a bank and/or a parent company with the appropriate credit rating, which provides an equivalent level of security for the ability to fulfil the contract. Such alternative documentation shall be attached as a part of the tender.

Teknisk og faglig kapasitet:

A. It is a criterion that the Tenderer has sufficient experience from similar projects. Similar projects include the supply of the service under competition to large public agencies/enterprises or large private company groups.

Documentation: List of the most important and most relevant deliveries during the past three years, including value, time period and contract authority. Documentation given or confirmed by the contract authority can be attached.

B. The Tenderer shall have sufficient capacity to adequately serve the Contracting Authority

Documentation: A chart depicting ownership/organisational structure shall be attached, as well as an overview of the Tenderer's total staffing divided into different departments and possible parent and subsidiary companies.

c. The Tenderer must have a well functioning quality assurance system for the services in order to handle situation where the tenderer experience failure in deliveries

Documentation: If the procedures mentioned in the Tenderer's quality assurance or quality control system comply with ISO standards or a comparable third party certification system, it is sufficient to produce a valid certificate with information regarding the availability of further details. Alternatively, an account of existing systems and procedures can be made in order to document that the criterion is fulfilled.
