Web based system for individual plans for municipalities in Sogn and Fjordane, including Helse Førde

The municipalities are responsible to honour the user's right to an Individual Plan (IP), this is in accordance with; The Health and Welfare Act, The Patients' Right Act.
The Norwegian Labour and Welfare Act (the NAV Act), the Act for Establishment and Implementation of Mental Health Care, the Act for Services in The Labour and Welfare Administration, the Act for Specialist Health Services etc., the Child Protection Act as well as regulations for Individual plans. The statutory obligation to name a co-ordinator shall apply regardless of whether the patient or user wants to utilise the right to an Individual Plan (HOL §7.2). The municipalities shall procure an annual right of use contract for a standard web based solution for Individual Plan. The right of use contract shall contain the right to use the web application. The web application shall be installed at the tenderer and the tenderer shall be responsible for the operation, maintenance, program development and support. Upgrades and new versions shall be included. The following municipalities are involved from the start: Bremanger, Eid, Flora, Førde, Gloppen, Luster, Songndal, Stryn and Årdal. Also the rest of the municipalities in the county will join the agreement. In addition, Helse Førde will participate.

Økonomisk og finansiell stilling:
The contracting authority will obtain information on the tenderer's economic and financial capacity via Proff Forvalt credit information company. It is therefore not necessary to submit this information. (All companies that have submitted their accounts to the Brønnøysund Register Centre will be registered in Proff Forvalt). Tenderers are asked to check whether such information is available in Proff (www.forvalt.no).

Tenderers who have not submitted their accounts to the Brønnøysund Register Centre, and are thus not registered in Proff Forvalt, or who do not fulfil the character requirement, can prove their economic and financial capacity with bank/parent company guarantees, relevant liability insurance and/or extracts from the annual accounts.

Minste nivå(er) av standarder:
The tenderer must have good economic and financial strength as regards their company in general and as regards this procurement's character.
A minimum character of «satisfactory» is required within two of the following criteria: liquidity, profitability and solidity, as described in Proff Forvalt's rating.
Teknisk og faglig kapasitet:
1. A portrayal of the tenderer's technical personnel or technical entities that the tenderer has at their disposal for the fulfilment of the contract, whether they belong to the company or not.
General overview of resource persons and experience competence for professionals/consultants that the tenderer possesses within the relevant disciplines.
2. List of the most important deliveries or services performed during the last 3 years, including information about the value, time, as well as the name of the public or private recipients with contact information for any collection of references.
3. Copy of certificate of environmental management system, according to ISO 14001, EMAS Miljøfyrtårn [the Environmental Lighthouse, Ltd] or Alternatively a portrayal of own environmental management system can be delivered.
4. Copy of certificate for the quality management system in accordance with Requirements in ISO 9001:2000 or Alternatively, an account of the tenderer's quality management system may be attached.
Minste nivå(er) av standarder:
1. Good implementation ability is required.
2. Good experience from equivalent assignments is required.
3. System and procedures that ensure low environmental impact with the implementation of the delivery are required.
4. A good and well functioning quality management system for the services that are to be delivered is required.
