The tender applies to the procurement of one x-ray laboratory with adapted generator and work station to Volda Hospital
Otherwise, the delivery must be inclusive of all components/accessories, assembly/installation and commissioning so that it functions as a whole based on the needs and requirements described.
Provision of the required training for all personnel who work in the affected units must be included.
The tender also covers the provision of services — and maintenance agreements where it will be important to clarify the need for service — and maintenance, and to clarify the annual operating costs expected after the warranty expires. Additionally, relevant technical courses.
Note that the qualification questions (in the CTM) are in some connections based on a function and performance descriptions as a specification method. It is therefore the tenderers' responsibility to submit specific and detailed tenders that cover the hospital's requests and requirements in an adequate and complete manner.

KK Tax certificate. The tenderer is required to have fulfilled obligations in terms of the payment of taxes. Documentation requirement. Certificate issued by a competent authority confirming that the tenderer has fulfilled obligations with regard to the payment of taxes. (In Norway: Certificate(RF 1244) from the tax collector and the municipal treasurer/collector of taxes, not older than six months). If the tenderer plans to use sub suppliers, tax certificates must also be submitted for these.

KK VAT certificate. The tenderer is required to have fulfilled its obligations in terms of the payment of VAT. Documentation requirement. Certificate issued by a competent authority confirming that the tenderer has fulfilled obligations with regard to the payment of VAT. (In Norway: Certificate (RF1244) from the tax collector and the municipal treasurer/collector of taxes, not older than six months). If the tenderer plans to use sub suppliers, VAT certificates must also be submitted for these.

KK — Company registration certiifcate. Requirement: The tenderer shall be a legally established company, sole trader or company under incorporation (SUS). Documentation Requirement. Established company: Certificate from the Register of Business Enterprises or certificate of registration in the professional register with regard to the legislation in the country in which the tenderer is established. Recently established/in the process of being established: Certificate from the Register of Business Enterprises, or if not available, the Memorandum of Association (AS).

Teknisk og faglig kapasitet:

KK Quality assurance system. Requirement. A good and well-functioning quality assurance system is required (ISO 9000 or equivalent). Documented by a certificate or other proof that substantiates the qualification requirement.

KK — Service Requirement: The tenderer shall have a service organisation that has good expertise and experience in the service/repair of the offered equipment. Documentation requirement: A description of the service department/organisation.

KK Reference. Requirement: Experience is required from equivalent assignments. Documentation requirement: List contracts in Europe, of the same type and extent. State the contact person, phone number and sales year for the last three years.
