The following shall be procured for Ålesund hospital, the Medical Department, Lung Section:
1 complete endoscopy rack including:
— 2 flexible bronchoscopes (large work channel),
— 1 thin flexible bronchoscope,
— 1 UL scope — EBUS.
The necessary accompanying equipment
In addition prices are wanted for a selection of EUS scopes as an option, as well as an option for the equivalent bronchoscope rack for Molde. This will not be included in the price evaluation.

KK Tax certificate. The tenderer is required to have fulfilled its obligations in terms of the payment of taxes. Documentation requirement. Certificate issued by a competent authority confirming that the tenderer has fulfilled his obligations with regard to the payment of taxes. (In Norway: Certificate(RF 1244) from the tax collector and the municipal treasurer/collector of taxes, not older than 6 months). If the tenderer plans to use sub suppliers, tax certificates for tax must also be submitted for these.

KK VAT certificate. The tenderer is required to have fulfilled its obligations in terms of the payment of VAT. Documentation requirement. Certificate issued by a competent authority confirming that the tenderer has fulfilled his obligations with regard to the payment of VAT. (In Norway: Certificate (RF1244) from the tax collector and the municipal treasurer/collector of taxes, not older than 6 months). If the tenderer plans to use sub suppliers, tax certificates for VAT must also be submitted for these.

KK - Company registration certificate. Requirement: The tenderer shall be a legally established company, sole trader or company under incorporation (SUS). Documentation Requirement. Established company: Certificate from the Register of Business Enterprises or certificate of registration in the professional register with regard to the legislation in the country in which the tenderer is established. Recently established/in the process of being established: Certificate from the Register of Business Enterprises, or if not available, the Memorandum of Association (AS).

Teknisk og faglig kapasitet:

KK Quality assurance system. Requirement. A good and well-functioning quality assurance system is required (ISO 9000 or equivalent). Documented by a certificate or other proof that documents the qualification requirement.

KK - Service Requirement: The tenderer shall have a service organisation that has good expertise and experience in the service/repair of the offered equipment. Documentation requirement: A description of the service department/organisation.

KK Reference. Requirement: Experience is required from equivalent assignments. Documentation requirement: List contracts of the same type and extent and Europe. State the contact person, phone number and sales year for the last 3 years.
