Petroleum Safety Authority Norway (PSA)

As part of the Petroleum Safety Authority's (PSA) project ‘Development in risk levels of Norwegian petroleum activities’ (RNNP) a questionnaire survey shall be carried out almost all personnel working on the Norwegian Continental shelf and at petroleum facilities on land. The questionnaire survey shall be carried out in the period 12 October to and including 22.11.2015, with reporting in February and March 2016 in accordance with the tender document.

Tax certificate — Requirement: The tenderer shall not be liable for tax or VAT. Documentation requirement: Tax Certificate. Issued either by the local tax office or the tax collector's office where the tenderer has their Head Office.
HSE requirement: The tenderer shall fulfil statutory HSE requirements. Documentation requirement: HSE declaration.
Company registration certificate — Requirement: The tenderer must be a legally established company. Documentation requirement: Company registration certificate.
Økonomisk og finansiell stilling:
Financing — Requirement: The tenderer must have the financial capacity to fulfil the contract/assignment. Documentation requirement: The accounts for the last two years, including the annual report with notes and the auditor's statement. Credit evaluation/rating, not older than 1 year, and which is based on the last known fiscal figures. The rating shall be carried out by an officially certified credit rating institution.

Teknisk og faglig kapasitet:
Competence and experience — Health, working environment and safety on the Norwegian Continental shelf and petroleum facilities on land — Analyse, evaluation and reporting of results of questionnaire forms within health, working environment and safety. Documentation requirement: The tenderer's competence profile. This means a general description of what competence and qualifications the tenderer can offer. Tenderer's experience profile. This means an overview of experience with equivalent assignments and then particularly the types of questionnaire surveys as named in the Requirements. The tenderer must provide a list of the most important deliveries during the past 3 years, including information on value, date and recipient (name, telephone number and e-mail address). State contracting authorities must be able to contacted if required.

Capacity — Requirement: Sufficient capacity to implement the assignment — The assignment involves the execution of the questionnaire survey based on both paper forms and electronic response, on behalf of many companies (facilities and plants) at differation locations and without the possibility to distribute links to electronic responses with the help of email. Documentation requirement: An overview shall be delivered of the relevant manpower at the tenderer, including any sub-contractors in addition to an overview of IT tools etc. as shall be utilised for the implementation of the assignment.

Quality assurance system — Requirement: A good and well-functioning quality assurance system is required for the services which shall be delivered. Documentation requirement: A statement regarding the tenderer's quality assurance system/quality management system or a copy of the system certificate issued by the accredited certification bodies or equivalent documentation.
