EEA 010-2015 Mobile C-arms for Bergen emergency doctor surgery

Bergen emergency doctor surgery would like to receive tenders for two mobile, identical, 'state of the art' C-arms with a flat panel detector. The offered arm shall be balanced out with a generator, x-ray tube and detector. The project shall be carried out during 2015, we envisage that the procurement and installation will take place in the last half of 2015. The C-arms shall mainly be used for orthopaedic procedures.
The C-arms will be used by radiographers and doctors for the repositioning and treatment of broken bones and dislocations on extremities, the removal of foreign objects, as well as for other minor surgical interventions in the same area. It must also be possible to use the C-arm for exposure, for example taking pictures post treatment. The offered apparatuses shall be digital.
Good access in the arm will be important, as it shall be used for treatment, for example, for plaster casts.
Emphasis will be put on how easy it is to turn and move due to on-site conditions. There must be good ergonomic solutions and it must be easy to operate.
The apparatuses will be in daily use. The operational time is divided between 2 shifts throughout the year. The 2 installations will have a slightly different rate of use.

Requirement: The supplier shall have their tax and VAT payments in order. Documentation: Tax and VAT certificate, issued by Altinn, the Tax office or the Tax Collection Office. The certificate shall not be issued more than six months before the tender deadline. Foreign suppliers are to provide certificates from the corresponding authorities in the countries where they are registered for tax. If the authorities in respective countries do not issue such authorisation, the supplier must provide a self-declaration form regarding whether there are arrears on taxes and if so, the amount of this. The self declaration must be signed.

Requirements: The supplier shall be a legally established company. Documentation: Company Registration Certificate, which has been issued no more than six months before the tender deadline. Confirmation of registration in a trade or business register as prescribed by the law of the country in which the supplier is established (applies to foreign companies).

Requirements: compliance with HSE regulations. Suppliers who shall perform work in Norway must meet the requirements for health, safety and environment that the Norwegian authorities at any time stipulate. Documentation: Appendix 4 HSE self-declaration form shall be completed. The declaration shall not be issued more than 6 months before the tender deadline.

Økonomisk og finansiell stilling:

Finance. Requirements: the company's solidity. The supplier must have sufficient economic and financial capacity to complete the contract. Suppliers can document their economic/financial capacity in the following ways:

— Credit rating report from companies that have a licence from Datatilsynet [the Norwegian Data Protection Authority] to operate a credit information company in Norway, possibly from a company with equivalent certification in a member country in the EU/EEA. The credit rating report must be based on updated accounting information.

— The last year's Annual Financial Statement including the board's annual report and the auditor's statement. Newly started companies that cannot present approved annual accounts can present the accounts that have been completed.

— Appropriate bank/parent company guarantees and/or relevant liability insurance.

The Contracting Authority will regard bank/parent company guarantees or relevant liability insurance as suitable if these documents cover the Contracting Authority's financial risk where the supplier does not have the financial or economic capacity to fulfil the contract. The contracting authority's financial risk means economic loss caused by non fulfilment of the contract. Parent Companies providing guarantees must document that they themselves comply with the requirement for 'the company's solidity'. The supplier is free to provide other information that can be seen as relevant for the assessment of whether the company's solidity requirement is fulfilled. The Contracting Authority reserves the right to obtain credit rating reports.

Teknisk og faglig kapasitet:

Requirement: Experience from similar assignments. Suppliers are required to have very good experience from similar contracts for the delivery of equivalent C-arms. Documentation: The supplier must complete Attachment 5 — Reference Form, concerning the 3 most important relevant contracts in the last 3 years, including their scope, value, time frame and the contracting authority or recipient. The reference places must have almost identical examination ranges and operation as the x-ray department at Bergen emergency doctor surgery.

Requirements for quality management. The assignment requires suppliers who have systems for quality management and effective flows of goods. Documentation (to be provided in the following ways): Description of the company's procedures regarding quality management, with emphasis on quality assurance, resource management, managements' responsibility, the manufacturing process and continual analysis and improvement. If the supplier holds an ISO 9001:2008 certificate (or newer version), such a certificate will be accepted as documentation of compliance with the qualification requirement.
