The aim of the procurement is to obtain treatment aids with the accompanying consumables for the Regional Unit for Treatment Aids with the accompanying health companies in Helse Midt-Norge.
The products and amount are described in Annex A, Specifications and Annex B, Price Form.
Tenders can be submitted for one or several lots. Tenderers shall clearly state which area the tender is submitted for. The contracting authority emphasises that tenderers shall also include all disposable equipment, reusable equipment, and consumables for the offered equipment. Information and prices area are to be specified in accordance with the annex.
The provision of the required training for all personnel who work in the affected unit must be included in the delivery.
The tender is for a framework agreement for a 2 -two- year period with an option for an extension for up to 1+1 year. The contracting authority would like tenders for apparatuses/equipment for use in home therapy treatment.

KK — 01 Tax certificates for tax and VAT. Tenderers are required to have fulfilled their payment obligations for tax and VAT. Documentation requirement. Certificate issued by a competent authority confirming that the tenderer has fulfilled his obligations with regard to the payment of tax and VAT. (In Norway: Certificate (RF1316) from the tax collector and the municipal treasurer/collector of taxes, not older than 6 months). If the tenderer plans to use sub suppliers, a tax certificate and VAT certificate must also be submitted for these, with the equivalent requirements for fulfilment of their obligations. The contracting authority draws attention to the fact that we will accept certificates issued in accordance with RF 1244, but draws attention to the fact that if these are used, 2-two- forms that equal RF 1316 shall be enclosed.

KK — 02 Company registration certificate. Requirement: The tenderer shall be a legally established company/sole trader or company being founded (SUS). Documentation Requirement. Established company: Certificate from the Register of Business Enterprises or certificate of registration in the professional register with regard to the legislation in the country in which the tenderer is established. Recently established/in the process of being established: Certificate from the Register of Business Enterprises, or if not available, the Memorandum of Association (AS). The Business Registration Certificate presented must not be more than 12 months old.

KK — 03 Requirement: Submit a self-declaration on health, environment and safety (HSE). Documentation requirement: See the attached self-declaration for HSE. This shall be filled in and enclosed with the qualification requirement. If a tenderer does not use the attached form for responding, the annex shall be recent (not older than nine months), as well as a justification as to why the tenderer has not used the contracting authority's attached form for a HSE self-declaration.

KK — 04 Requirement: Confirmation that the company is not currently the subject of debt composition or bankruptcy negotiations. Documented through a self-declaration, bank confirmation, auditor's report, etc.

KK — 05 Requirement. Enclose the company's most recent annual report to Brønnøysund Registre Centre and new information of relevance to the company's fiscal figures. Documentation requirement: Annual report to the Brønnøysund Register Centre, as well as the last year's profit and loss accounts for the tenderer's company.

KK-06 Requirements: The tenderer shall have sufficient capacity to carry out the assignments that arise during the agreement period and in accordance with the necessary progress to safeguard the contracting authority's interest.
