Requirement to submit declaration in accordance with Regulations attachment 2 on HSE (National requirement).
Appendix 2 "Information about the supplier" must be completed and attached.

The company's most important deliveries over the last 3 years, including their value, time frame and recipient (regarding goods and service contracts)
Confirmation from the Labour Inspectorate Authority that the operations are approved in accordance to FOR-2009-09-10, paragraph 1173, Regulation on approved health companies etc. should accompany the tender. It is assumed that such approval will be maintained throughout the contract period.
