K 01 Tax certificate - Requirement: The tenderer is required to have fulfilled its duty regarding the payment of tax. Documentation requirement - tax: Tax certificate. In Norway this is issued by the tax collection office. Tenderers from other countries should attach equivalent certificates from the countries in which they are established. The certificates should be issued on Form RF-1244 from the Directorate of Taxes or another form that gives the same information. Any arrears must be explained and justified. The certificate shall not be older than 6 months calculated from the deadline for receipt of applications.

K 02 VAT certificate - Requirement: The tenderer is required to have fulfilled its duty regarding the payment of VAT. Documentation requirement - VAT: Certificate for paid VAT. In Norway this certificate is issued by the tax office. Tenderers from other countries should attach equivalent certificates from the countries in which they are established. K 03 Legally-established company - Requirement: The tenderer is required to have a legally-established company. Documentation requirement - Legally-established company: Certificate from the Register of Business Enterprises. Certificate of registration in the Professional registry in accordance with the laws of the country where the tenderer is established. The certificate should not be older than 6 months calculated from the submission date for the tender.

K 04 Health, safety and environment - Requirement: It is a requirement that the tenderer satisfies the requirements in the regulation on systematic health, safety and environment work within the organisation. The organisation must be legally organised in regard to the applicable taxes - and working environment regulations in regards to employees professional and social rights. Documentation requirement - HSE: Filled in and signed Appendix - HSE Declaration Form.

Økonomisk og finansiell stilling:
K05: The tenderer's economical and financial standing Requirement: There is a requirement that the tenderer has an economical and financial solidity which gives the contracting authority assurance that the supplier will be economically viable throughout the duration of the agreement. It is a requirement that the tenderer has positive equity. A tenderer can support themselves financially and economically by having other companies fulfil this qualification requirement. This applies regardless of the legal association between the tenderer and the company/companies. If the tenderer wishes to support themselves on another companies economical and financial solidity, documentation must be submitted from the the tenderer and other company in regard to point - Documentation requirement Accounts. Furthermore, the declaration of obligation with the tender documents should be completed and attached with the tender. Documentation Requirement - Accounts: Reported accounts for the last two financial years with the audit report.

Teknisk og faglig kapasitet:
K06 Tenderer's experience: It is a requirement that the tenderer has sufficient experience. The Contracting Authority will here assess the tenderer's experience with delivery of the performance the competition comprises in regard to: - Volume/value of previous deliveries. The size of the recipient of previous deliveries, hereunder to which degree the recipient is of comparable size and complexity to the regional health authorities. For tenders for lots: The tenderer's experience will be assessed as regards volume/value for the part of the procurement that the tenderer has submitted a tender for, as well as the size of the regional health company the tender was for. A tenderer may rely on the experience of other companies to fulfil this qualification requirement. This applies regardless of the legal connection between the tenderer and the company/companies. Documentation requirement: Reply form experience, filled in by the tenderer.

K07 Ethically responsible company. Tenderers are required to have an ethically responsible company. Documentation requirement - Ethically responsible company. If a tenderer has been legally convicted in the last 5 years for corruption, fraud or whitewashing of money - or found guilty in criminal offences regarding occupational conduct - the tenderer must account for the conviction and case in a separate document.
