The contract is from 1.6.2013 to 31.5.2015 with an option for the awarding authority to extend the contract for a further 1 + 1 year on unchanged terms.

Vilkår for deltakelse
Egnethet til å utøve yrkesvirksomheten:
(1) Company registration certificate.
(2) Evidence of enrolment on the professional or trade register, as required by national law, in the country where the company is established.
(3) Value added tax certificate (National requirement).
(4) Company tax certificate (National requirement).
(5) Self-declaration in compliance with Appendix 2 (HSE) of the Norwegian procurement regulations (National requirement).
(6) Other documentation required (please state below).
Supplier shall have his tax and VAT payments in order.
Documentation requirement: Tax certificate for tax and VAT respectively, not older than 6 months. The certificates can be obtained from the local tax collection office or the tax office. Foreign suppliers shall enclose certificates from equivalent authorities as the Norwegian.

Suppliers shal enclose a HSE self-declaration (see annex 3). Documentation requirement: A signed HSE declaration.
Suppliers shall be a legally established company. Documentation requirement: Norwegian companies: Company registration certificate. Foreign suppliers: Proof that the company is registered in a trade register or company register as mandated in the law in the country where the supplier is established.

Økonomisk og finansiell stilling:

Credit rating, not older than 3 months based on the last known accounting figures. The entire credit rating shall be enclosed and it shall include the key figures for the last three accounting years. The rating shall be carried out by a publicly certified credit rating insitution. The awarding authority reserves the right to obtain its own credit rating if necessary.

Minste nivå(er) av standarder: Suppliers shall have the financial capacity to fulfil the contract.
Teknisk og faglig kapasitet:
(1) Other documentation required (please state below).
A description of the supplier's technical personnel that the supplier will use for fulfilment of the contract whether they belong to the company or not.
A list, with a short description, of the most important deliveries or services provided in the last three years, including information on value, date, as well as the name of the public or private recipient.
A description of how the supplier ensures that the EU-directive 93/42/EØF (Directive for medical equipment) is complied with (as regards a declaration of conformity, documentation on the execution of the work, marking technical tooth work etc.), also for any sub suppliers as well as information of any certifications.

A good ability to fulfil the contract is required.
Experience is required from equivalent contracts as regards the delivery of technical tooth goods/works.
A good and well functioning quality assurance system is required for the products/services that shall be provided.
