We ask for a tender for a full conventional x-ray laboratory for imaging injuries . The laboratory shall not be used for interventional radiology. Therefore there is no requirement for fluoroscopy functionality or C-loop solution.
The laboratory will be the department's imaging injuries/acute laboratory. There are up to 125 examinations performed per day. There is therefore great demand for functionality, capacity, ergonomics and user-friendliness Good functionality for investigation of small bones, inserted projections and chest X-ray is therefore emphasized .
X-Ray Laboratories shall be installed at the Departments of Medical Imaging, Namsos and Levanger Hospitals.

KK - 01 Tax certificate. It is required that the tenderer has fulfilled its obligations in relation to the payment of taxes. Documentary requirement. Certificate issued by the competent authority certifying that the tenderer has fulfilled its obligations relating to the payment of taxes. (In Norway: Certificate (RF 1244) from the tax collector as well as municipal treasurer/treasurer not older than 6 months.) If the tenderer plans to use subcontractors, tax certificates must also be submitted for them.

KK - 02 Tax Certificate for VAT. It is required that the tenderer has fulfilled its obligations in relation to the payment of VAT. Documentation Requirements. KK - 03 Certificate of registration - Requirements: Tenderer shall be legally established company / sole trader or company under establishment (SUS). Documentary Requirements. Established companies: certificate from business register or a certificate of registration in the professional registry cf law of the country where the tenderer is established. Newly established / being established: Certificate from the Register or if this is not available, memorandum of association (AS)

KK - 04 Requirements: to present a declaration of health, safety and environment (HSE). Documentary requirement: Completed self-declaration attached.

Økonomisk og finansiell stilling:

KK - 05 Requirements: Verify that you are not in on-going debt agreement or bankruptcy proceedings. Documented by a personal statement, bank verification, audit report etc.

KK - 06 requirements. Attaching company's last annual accounts to Brønnøysundregisteret and any new information of relevance to the company's accounting figures. Documentary requirement: Annual report to the Brønnøysund Register.

Teknisk og faglig kapasitet:

KK - 07 Requirements: A good and well-functioning quality assurance system is required. (ISO 9000 or equivalent). Documented by certification or other evidence documenting the eligibility requirement.

KK - 08 Requirements: Tenderer shall have A good and service organisation that has expertise and experience in service / repair of equipment of the equipment tendered. Documentary requirement: Description of the service department / organisation with cv.

KK - 09 Requirements: Experience from similar assignments. Documentary requirement: List of deliveries of the same type and scope within Europe. Supplied with the contact person, phone number and sales year for the last 3 years.
