1. Company registration certificate.

2. Value added tax certificate (National requirement).

3. Company tax certificate (National requirement).

4. Self-declaration in compliance with Appendix 2 (HSE) of the Norwegian procurement regulations (National requirement).

— Service providers shall meet the legal requirements in Norway within health, safety and environment (HSE);

— Service providers shall have a legally established company.

Økonomisk og finansiell stilling:

Qualification requirements:

— Service providers shall have sufficient financial capacity to perform the contract.

Documentation:

— The company's two latest annual financial statements, including notes and auditor's report. If no annual report for 2012 is completed at the time of submission of the tender, preliminary financial statements shall be submitted;

— Credit rating from an approved credit institution;

— If service provider relies on other providers to meet this requirement, documentation shall also be submitted for these suppliers, including any parent companies.

Teknisk og faglig kapasitet:

A list of the principal deliveries effected, or main services provided, by the company, in the past three years, with the values, dates and recipients (for supply and service contracts).

A statement of the quality assurance/quality control system operated by the company.

Qualification requirements:

— Service provider shall have good execution ability, including sufficient capacity and expertise;

— Service provider shall have experience from similar deliveries;

— Service provider shall have a quality assurance system tailored to the service.

Further documentation requirements:

— An account of service provider's average workforce and number of administrative employees over the last three years;

— Information on education and professional qualifications in service provider's company, by means of one or more competence matrix;

— Any documentation of sub-contractors' experience.
