Execution of the research questionnaire 2013 RNNP

Petroleum Safety Authority Norway (PSA)

As part of The Petroleum Safety Authority Norway's project "Utvikling i risikonivå norsk petroleumsvirksomhet" (Trends in Risk Level in the Petroleum Activity - RNNP) a questionnaire shall be carried out amongst all personell working on the Norwegian Continental shelf and on petroleum installations on land. The distribution and collection of questionnaires shall be carried out in the period 14.10.2013. to and including 24.12.2013, with reporting in 2. and 3.2014 in accordance with the tender document.

Økonomisk og finansiell stilling:

- Annual accounts for the last 2 years including management reports and auditors' notes.

- Credit evaluation/rating, no older than 1 year, based on the last known accounting figures. The rating shall be carried out by a publically approved credit evaluation company.

1. Tenderer's competence profile. This means an overview of the competence the tenderer can provide.

Tenderer's experience profile. This means an overview of experience from equivalent assignments, especially from the type of research questionnaires which are requested for this assignment.

Overview of supplier's most relevant assignments in the last 3 years, including statement of the assignments value, date and recipient (name, telephone and e-mail).

2. Overview of manpower, IT tools and potential sub-contractors to be used for carrying out the assignment.

3. Account of the service provider's quality assurance/control system

and/or

A copy of the system certificate issued by accredited certification bodies or equivalent documentation.

Minste nivå(er) av standarder:

1. Competence and experience

Tenderer must document high professional competence and experience from:

- occupational health and safety (working environment) and safety regarding major accidents on the Norwegian Continental Shelf and onshore petroleum sites

- analysis, evaluation and reporting of results of questionnaires on working environment and safety.

2. Sufficient capacity to undertake the assignment.

The questionnaire-based investigation will be carried out using both paper forms and electronic replies, actoss many companies in different locations and without the possibility to distribute links for electronic responsed via e-mail.

3. A good and well functioning quality assurance system is required for the service which shall be provided.
