Collection, transport and final treatment of infectious/biohazardous waste

Tenders shall include the collection and transport of waste and the treatment/final disposal of the waste. This notice is for the following types of waste:
1. Amputates.
2. Medical waste, i.e.
a. Infectious waste, i.e. bandages, blood remains, surgical waste etc.
b. Medical remains (antibiotics, cytostatica, antiviral agents etc. in pure form, diluted and as remains in packaging, infusion bags etc.)
c. Pricking/cutting waste.
Tenders shall also include the hire of collection units, as well as the maintenance and cleaning of the hired collection equipment. 5m3 steel containers are currently used.

Oslo University Hospital would like tenders for three different contract lengths: Alternative 1: 2 year contract with an option for a 1+1 year extension. Alternative 2: 4 year contract with an option for a 2 year extension. Alternative 3: 8 year contract without an extension. Oslo University Hospital will, after an assessment, choose the length that is most appropriate for the hospital. The new contract shall come into force and be operative 1.1.2014. (NT Ref:297604)

Vilkår for deltakelse
Egnethet til å utøve yrkesvirksomheten:

(1) Company registration certificate.

(2) Value added tax certificate (National requirement).

(3) Company tax certificate (National requirement).

(4) Self-declaration in compliance with Appendix 2 (HSE) of the Norwegian procurement regulations (National requirement).

(5) Other documentation required (please state below).

If a tenderer has, in the last five years, been legally convicted for corruption, fraud or whitewashing money, or been found guilty in punishable offences concerning professional conduct, an account must be given of the conviction and case.

Minste nivå(er) av standarder:

Tenderers shall have sufficient economic and financial solidity to fulfil the contract obligations.

- Tenderers shall enclose the annual accounts for the last two accounting years, together with the auditor's annual report.

- Tenderers can be credit rated.

- Newly started companies shall enclose the accounts and auditor's report for the period that the company has been in operation.

- A bank guarantee that gives the same security for contract fulfilment can be given.

Tenderers shall have good qualifications and the capacity related to the contract obligations.

o A short description of the tenderer's personnel and technical units that the tenderer can use for fulfilment of the contract.

o Documentation of education and professional qualifications for the company in general.

o Documentation of information on education and professional qualifications for the or those persons that are responsible for carrying out the service (CVs are to be enclosed).

o List of the most important and most relevant contracts in the last three years, including information on value and date, as well as the name of the public or private recipient. Reference persons with contact details shall be given. These will be contacted to the degree that is deemed appropriate.

o If a tenderer will use sub suppliers, a binding statement shall be enclosed that shows that the tenderer can use the necessary resources for the entire contract period.

o An account of the tools, machines, instruments, materials or technical equipment that the tenderer can use for fulfilment of the contract/assignment

o An account of the tenderer's quality assurance system/control system.

o Documentation that the vehicles and drivers are certified to drive this waste.

Competence and systems are required that ensure that the service can be carried out with a low impact on the environment and a high degree of safety. This applies to both transport and work.

o Tenderers shall document their environmental management measures/environmental management system.
