Nordland Hospital shall enter into a contract for waste and sanitation services. This includes the services stated in the tender documents and price form as follows:
The collection of waste from containers and collection points.
Hire of suitable waste containers and containers.
Emptying or towing "self-owned" containers to reception centres for waste for emptying.
Collection of various special waste from Bodø, Gravdal and Stokmarknes.
Collection of biological waste and cytostatica from the hospitals in Bodø, Gravdal and Stokmarknes - for transport to a certified incineration plant. Sub contracts will be signed per location for NLSH Bodø, NLSH Lofoten (Gravdal) and NLSH Vesterålen (Stokmarknes) respectively with the surrounding areas.
One total service provider is expected per location. Tenderers must submit a complete tender per location.
Furthermore option prices are requested for the hire of various containers and for sludge suction services. The probability of call-offs onfor the option is %.
If a tenderer does not have an offer for these options, it is not an absolute requirement to give prices for this.
The start of the new contract is three months after the contract date, rounded up to the next month.

Vilkår for deltakelse
Egnethet til å utøve yrkesvirksomheten:
(1) Company registration certificate;
(2) Value added tax certificate (National requirement);
(3) Company tax certificate (National requirement);
(4) Self-declaration in compliance with Appendix 2 (HSE) of the Norwegian procurement regulations (National requirement);
(5) Other documentation required (please state below);

— A satisfactory quality assurance, control and internal control system. Relevant documentation is to be enclosed.
— Service providers are to have prepared ethical guidelines, which describe the content and enforcement of them. A copy of the company's ethical guidelines shall be enclosed.
— Service providers are to be environmentally certified in accordance with ISO 14001, Miljøfyrtårn or equivalent. A copy of a certificate is to be enclosed.
— Service providers are not to have managers, owners, or others in an equivalent position, for example, management positions that have the authority to represent the organisation, make decisions or in another way control, who have been convicted for the conditions mentioned in the Public Procurement Regulations §20-12 3.

— Service providers are to enclose a declaration stating that the persons in key positions have not been legally convicted in the mentioned areas. The document shall be signed by the chairman of the board or the manager.
Økonomisk og finansiell stilling:

— Minimum rating of satisfactory solidity/equity. The last 2 years auditor approved annual accounts shall be enclosed.
— Service providers shall have suficient liability insurance to cover claims from the awarding authority or third party. The insurance sum shall not be less than 100 G. A valid insurance certificate/contract is to be enclosed.
Teknisk og faglig kapasitet:
Service providers shall have public certification/follow current laws/regulations.
— Service providers and any cooperation partners must have the necessary public certifications for waste handling (documentation is to be enclosed)
— Licence in accordance with the Pollution Act from the County Governor where relevant as confirmation that the Pollution Act and Waste Regulations are followed.
Very good experience is required from equivalent contract.
A list is to be enclosed of the most relevant deliveries or service performed in the last 3 years, including information on value, date and the name of the public or private recipient. Services to public authorities or private companies shall be documented by certificates issued or confirmed by the relevant recipient. If such certificates do not exist, a self-declaration can be accepted from the service provider. The self-declaration shall include the necessary information on each delivery/contract and it shall include the name of the awarding authority's contact person, his position, telephone number and e-mail address.
