Vilkår for deltakelse
Egnethet til å utøve yrkesvirksomheten:

— Tax certificate.

— VAT certificate.

The certificates shall be issued on form RF-1244 from the chief municipal treasurer. The certificates shall not be older than 6 months. A reason must be given for any arrears.

— Foreign tenderers must enclose equipment confirmation that public taxes have been paid in the country where the tenderer has his place of business.

Økonomisk og finansiell stilling:

— Tenderers shall enclose the annual accounts for the last two accounting years, together with the auditor's annual report.

— Tenderers can be credit rated.

— Newly started companies shall enclose the accounts and auditor's report for the the period that the company has been in operation.

— A bank guarantee that gives equivalent security for contract fulfilment can be given.

Teknisk og faglig kapasitet:

— Short description of the tenderer's personnel and the technical units that the tenderer can use for fulfilment of the contract.

— Documentation with information on education and professional qualifications of the persons who will be responsible for carrying out the service (CVs are to be enclosed).

— List of the most important and most relevant contracts in the last three years, including information on value and date, as well as the name of the public or private recipient.

— If a tenderer will use sub suppliers a binding statement shall be enclosed that shows that the tenderer can use the necessary resources for the entire contract period.

— Account of the tools, machines, instruments, materials or technical equipment that the tenderer can use for fulfilment of the contract/assignment.

— Account of the tenderer's quality assurance system/control system. Tenderers must document how they fulfil the quality requirements in "Volume 4 EU Guidelines to Good Manufacturing Practice Medicinal Products for Human and Veterinary Use. Part I. Annex I.".

— Copy of a system certificate issued by an accredited certification organ or equivalent documentation.

— As the awarding authority does not have storage capacity, tenderers must guarantee weekly deliveries to both Radiumhospitalet and Rikshospitalet.

Kontraktutførelse
Juridisk form som skal antas av gruppen av økonomiske operatører som kontrakten skal tildeles til:

— Norwegian tenderers shall enlose an updated company registration certificate.

— Foreign tenderers shall enclose a certificate(s) for legal registration in the establishment country.

— HSE self-declaration in accordance with the regulation's annex 2, tenderers shall fulfil, or will fulfil upon an award of contract, legal requirements in Norway within health, environment and safety. The self-declaration shall be co-signed by one of the employees in the company.
