Vilkår for deltakelse
Egnethet til å utøve yrkesvirksomheten:

(1) Company registration certificate.

(2) Value added tax certificate (National requirement).

(3) Company tax certificate (National requirement).

(4) Self-declaration in compliance with Appendix 2 (HSE) of the Norwegian procurement regulations (National requirement).

Requirement: Certified rehabilitation company, cf. the FOA §3-12: Self-documentation that the company fulfils the requirements for the entity in the FOA §3-12.

Økonomisk og finansiell stilling:

Requirement: The company's solidity.

The service provider must have sufficient economic and financial capacity to fulfil the contract.

Documentation:

Bergen municipality will obtain information on the service provider's economic and financial capacity via Proff Forvalt credit information company. It is therefore not necessary to provide this information. (All companies that have sent their accounts to the Brønnøysund Register Centre will be registered through Proff Forvalt.) Tenderers must check whether such information is available in Proff Forvalt (www.forvalt.no).

Teknisk og faglig kapasitet:

Requirement: Ability to fulfil the contract - personnel. Service providers are required to show a good ability to fulfil the contract in accordance with the awarding authority's need. Service providers are required therefore to have sufficient capacity to fufil the assignment.

Documentation: Overview of the company's total manpower.

Requirement: Routines for quality assurance.

The assignment requires the service provider to have routines and systems that ensure a low impact on the environment when providing the service.

Documentation (shall be given in one of the following ways):

Description of the company's routines and systems connected to the choice and handling of chemicals, traceability of raw goods, recycling/minimizing of packaging.

If the routines are described in the company's quality assurance or environment management system in accordance with ISO 14001 or equivalent third party verified system, the valid certificate and acopy of the relevant routines shall be enclosed.

Quality assurance requirements.

The assignment requires the service provider to have systems for quality assurance and an efficient flow of goods.

Description of the company's routines for quality assurance, with an emphasis of quality assurance, resource management, the managment's responsibility, manufacturing process and continual analysis and improvement. If the service provider has an ISO 9001:2000 certificate, it can be enclosed.

Requirement: Experience from similar assignments.

Service providers are required to have good experience from similar assignments.

Documentation:

Service providers must fill in annex 4 - reference form on the 3 most important relevant contracts in the last 3 years, including their extent (number of manlabour months), value, date and awarding authority or recipient.
