Oslo municipality c/o the Health Department shall enter into a framework agreement with one supplier for dental goods and services for the awarding authority's dental clinics in Oslo, Norway.

Beskrivelse av opsjoner:
The agreement will apply from 1.2.2013 until the 31.12.2014 with an option for the awarding authority to extend the agreement for a further 1+1 years.

Vilkår for deltakelse
Egnethet til å utøve yrkesvirksomheten:
(1) Company registration certificate.
(2) Evidence of enrolment on the professional or trade register, as required by national law, in the country where the company is established
(3) Value added tax certificate (National requirement)
(4) Company tax certificate (National requirement)
(5) Self-declaration in compliance with Appendix 2 (HSE) of the Norwegian procurement regulations (National requirement)

The service provider shall have his tax and VAT payments in order. Foreign service providers shall enclose certificates from equivalent authorities to the Norwegian requirements.
The service provider shall be a legally established company: Foreign companies: Proof that the company is registered in a trade register or company register as mandated in the law in the country where the service provider is established.

Credit rating, not older than 3 months, based on the last known accounting figures. The entire credit rating report shall be provided and it shall include the key figures for the last three accounting years. The rating shall be carried out by a publicly certified credit rating institution. The awarding authority reserves the right to obtain its own credit rating if necessary.

Description of the company's technical personnel that the supplier will use to fulfill the contract, whether they are employed by the company or not.
A list of the principal deliveries effected, or main services provided, by the company, in the past three years, with the values, dates and included with the name of the public or private sector recipients.
A statement of the quality assurance/quality control system that verifies that the EU-directive 93/42/EEC (directive for medical devices) is met and also applies for subcontractors.

Requires a good and efficient quality assurance system for the products that will be delivered.
