(4) Self-declaration in compliance with Appendix 2 (HSE) of the Norwegian procurement regulations (national requirement).

(1) The company’s most recent annual report including new information of relevance to the company’s accounting figures.
(2) Presentation of the company’s annual accounts, or extracts thereof.
Teknisk og faglig kapasitet:
(1) Other documentation required (please state below).
1. A short account shall be given of the necessary competence in Norwegian food production (conventionally and organically produced) and the competition situation in the Norwegian market. Any competence in work with labels for food is to be given.
2. A short account shall be given of how the whole will be kept within the marketing services.
3. A short account shall be given of the organisation of the work at the service provider and contact surface with Matmerk. If tenders are given for more than one of the areas/products, a description of how this will be handled at the service provider must be enclosed.

4. Documentation shall be given of the work processes that shall be carried out for working out agreed assignments for customers.
5. A reason shall be written as to why the tender comprises the chosen area(s)/product(s).
6. Documentation shall be enclosed of 3 assignments that are relevant for deliveries to Matmerk, including the assignment’s financial amount, when the assignment was carried out and references.
