EEA 034-2011 Hellen school - Renovation and extension

Bergen kommune

Hellen school is currently a primary school with 364 pupils from the 1st – 7th years. Hellen school shall be renovated and extended to a school for approx. 400 pupils. A proposal has been worked out for a development plan in accordance with the requirements from the department for building applications and private plans. This is expected to be formally approved during autumn 2011, parallel with the approval of the framework application. The assignment consists of the construction of the extension and upgrading Hellen school. The building will have an extension towards the north, on both the west and east wings. In addition the largest part of the existing buildings is to be adapted and altered.

(4) Self-declaration in compliance with Appendix 2 (HSE) of the Norwegian procurement regulations (National requirement).
Teknisk og faglig kapasitet:

1. The company shall have experience from similar assignments which have been carried out in a satisfactory way. Similar assignments mean:
Extent of comparable character and size as well as degree of difficulty.
Documentation:
The company’s relevant contracts in the last five years, including their value, date and recipient. A reference person at the recipient must be given.
In addition to the received documentation, the awarding authority can emphasize its own documented experiences with the company.
If a company does not have the necessary qualifications itself, but will rely on other contractors in order to be qualified, equivalent documentation for these must be enclosed. In addition a binding statement must be enclosed in accordance with point 3.3.1 and the attached template.

2. A well functioning quality system adapted to the contract’s contents.
Account of the company’s quality assurance system/control system. The account must at a minimum include:
1. A general description of the system’s contents including an overview of control plans and check lists which are relevant for this contract.
2. A description of when and how the system was implemented in the company and how often the system is updated as well as.
3. A description of how the system will be used to ensure quality in this contract.
If a company does not have the necessary qualifications itself, but will rely on other contractors to be qualified, equivalent documentation must be enclosed for these. In addition a binding statement must be enclosed in accordance with point 3.3.1 and the attached template.

3. Qualifications are required equivalent to development class 3 for being responsible for work in the following fields:
K200-Demolition.
— Ground work,
— Concrete work.
K230-Joinery, demolition.
K300-Plumbing work.
K360-Air treatment.
K400-Fire alarm installations.
Qualifications are required equivalent to development class 2 for being responsible for work in the following fields:
K200 – Outer roof.
— Steel work.
K240 Masonry.
— Floor laying work.
K600- Lift installations.
Qualifications are required equivalent to development class 1 for being responsible for the following works:
K700- Site gardening.
Central documentation for the relevant works, alternatively documentation showing that local certification will be achieved for the relevant works.
If a company does not the necessary qualifications itself, but will rely on other contractors to be qualified, equivalent documentation must be enclosed for these. In addition a binding statement must be enclosed in accordance with point 3.3.1 and the attached template.

4. The offered organisation and personnel shall have satisfactory competence and the ability to fulfil the contract. The key personnel shall be able to speak and write good Norwegian.
Organisation plan for the assignment which shows the offered personnel in the various key positions i.e. management in the project down to site manager.
CVs for the offered key personnel in the organisation plan. The CVs shall include experience, age, education, language abilities, relevant reference assignments and which role the person had in these assignments. If the contractor shall be the main company/SHA coordinator, relevant competence shall be included in the CV.
